Shedding on a GLP-1 is rarely just one thing

Search "does GLP-1 cause hair loss" and every answer says the same sentence: it is telogen effluvium, it is triggered by rapid weight loss rather than the drug itself, and it grows back. That is broadly right12 — and for a lot of women it is also incomplete in a way that costs months.

The reason is that the years when women are most likely to start a GLP-1 are the same years when two other things start thinning hair: the perimenopausal hormonal transition4, and androgen- or iron-driven thinning that was already underway quietly. A GLP-1 does not switch those off. It adds a shed on top of them. So the honest question is not "is my hair loss from the medication" — it is "how much of it is, and what is the rest." This is general education, not a diagnosis; hair loss is one of the things worth taking to a clinician rather than a search bar.

What medication-linked shedding actually looks like

Telogen effluvium is a synchronising event. A metabolic stressor — rapid weight loss, a lower calorie intake, less protein than the body was used to — pushes an unusually large share of follicles out of the growth phase at the same time, and they let go together a couple of months later12. Its signature is specific:

- **It is diffuse.** The whole scalp thins at once. It does not carve out a part line or a hairline. - **It is a shed, not a thinning.** Hair comes out — in the shower, in the brush, in handfuls — with a small white bulb at the root. - **It is late.** It starts roughly two to four months *after* the weight loss got fast, which is why so many women blame the wrong month's dose change. - **It is self-limiting.** The follicle is not damaged. Shedding slows once weight loss slows, and density rebuilds over months.

If that is the whole picture, the treatment is mostly patience plus removing the amplifiers — enough protein, enough total calories, and a rate of weight loss that is not outrunning what the rest of the body can support. Not all GLP-1 users shed, and the rate of loss matters more than which molecule you are on2.

What perimenopausal thinning looks like instead

This one is not a shed, and it is the one most often misfiled as "the medication."

Through the menopause transition, falling estrogen shifts the balance of hormonal signalling at the follicle, and hair moves toward a female-pattern distribution: the part line widens, the crown loses density, the hairline holds4. It is gradual rather than dramatic. There is no handful in the shower. And unlike telogen effluvium it does **not** resolve on its own when weight stabilises — waiting it out is precisely the wrong move, because female-pattern loss responds best when it is treated early.

The practical tell: **a shed that ends is telogen effluvium; a part that keeps widening is not.** If density is still falling six to nine months after weight loss levelled off, the GLP-1 has stopped being a sufficient explanation.

When androgens are the driver

For women with PCOS, elevated androgens can drive scalp thinning in the same female-pattern distribution, and it often predates the GLP-1 by years6. Weight loss can improve the underlying metabolic picture, but the thinning has its own trajectory and its own treatments. The two labs that settle most of it

This is the part that gets skipped, and it is the cheapest information available.

**Ferritin.** Iron deficiency is meaningfully associated with non-scarring hair loss in women3, and the women most likely to be low are exactly the ones on a GLP-1 in midlife: heavy or irregular perimenopausal bleeding on one side, sharply reduced food intake on the other. Ferritin can be low enough to affect hair while a standard haemoglobin looks entirely normal — so "my blood count was fine" does not rule it out.

**Thyroid.** Thyroid dysfunction is a standard part of the hair-loss differential and is several times more common in women5.

What actually changes the outcome

- **Protein and total intake.** Appetite suppression is the point of the medicine, which makes under-eating protein the easiest mistake to make on it. This is the single most modifiable amplifier. - **The rate of loss.** Very fast loss is the trigger12. Slowing the pace — including via dose — is a real conversation to have with a prescriber rather than a failure. - **Testing rather than assuming.** Ferritin and thyroid, before spending months on supplements aimed at the wrong cause. - **Time, honestly counted.** Regrowth is measured in months, and new growth arrives as short baby hairs along the part before length returns.

What does not help: guessing which cause it is from a photo, or switching molecules on the assumption that another GLP-1 will not do it.
