Clinical oversight and labs carries the most weight because midlife is when thinking “it's just menopause” is most expensive. Thyroid disease is several times more common in women, ferritin can sit low enough to matter while a blood count reads normal, and both look like the transition from the outside. A program that measures is doing something a program that titrates cannot.
Hormonal suitability is the factor that asks whether the hormonal picture is handled at all, or routed around. Ongoing support is weighted because midlife weight is a multi-year problem and a program built for a twelve-week sprint answers a different question.

What the evidence actually says about the transition
The midlife metabolic shift is measurable, not imagined: across the menopause transition body composition moves toward more fat and less lean mass, and fat redistributes toward the abdomen even when the scale barely moves. Greendale et al., JCI Insight (2019) tracked exactly that through the SWAN cohort.
On whether these medicines still work: a study in postmenopausal women found weight-loss response to semaglutide was comparable whether or not they were using hormone therapy — Hurtado et al., Menopause (2024). A 2025 review in Current Opinion in Obstetrics & Gynecology summarises the current evidence for GLP-1 use in perimenopausal and postmenopausal women, and Palacios et al., Climacteric (2024) covers obesity management in menopause more broadly.
