How much does compounded semaglutide cost per month?
Across the 22 telehealth programs we review that publish a firm month-to-month price, compounded semaglutide costs $129 to $297 a month, with a median of $177. Half of them charge between $150 and $214. Compounded tirzepatide runs higher: $119 to $349, median $239. First-month offers and prepaid plans can look much cheaper than what you will pay from month two.
Those figures come from the price each program shows on its own website, as recorded in our provider reviews; each review shows the month it was last updated. We used the standing month-to-month price: what you pay once any first-month discount is over, billed once a calendar month, without committing to a multi-month prepayment. That is the number most women actually end up paying, and it is rarely the number in the ad. Programs that publish only a "from" price, bill every 4 weeks, or price a membership and the medication separately are left out of the index.
| Compounded semaglutide, standing monthly price | Programs | Share |
|---|---|---|
| Under $150 | 6 | 27% |
| $150 to $199 | 10 | 45% |
| $200 to $249 | 3 | 14% |
| $250 to $299 | 3 | 14% |
| $300 or more | 0 | 0% |
The single most common price is $179 a month. Of the 22 programs, 4 charge exactly that, which tells you something about how the market sets prices: many programs anchor to the same round figure and compete on what is bundled around it rather than on the medication itself.
How do compounded tirzepatide prices compare?
Tirzepatide is the compound in Mounjaro and Zepbound, and it costs more across the board. Of the 21 programs with a clear monthly tirzepatide price, the median is $239, and 6 of them (29%) charge $300 or more.
| Price point (standing monthly price) | Semaglutide | Tirzepatide |
|---|---|---|
| Lowest | $129 | $119 |
| 25th percentile | $150 | $199 |
| Median | $177 | $239 |
| 75th percentile | $214 | $329 |
| Highest | $297 | $349 |
| Programs with a clear price | 22 | 21 |
The gap between the two medians is $62 a month, or $744 a year. Whether that is worth paying is a clinical question as much as a financial one. Tirzepatide produced more weight loss than semaglutide in a head-to-head trial in adults with obesity1, and some women tolerate one better than the other. Our semaglutide vs tirzepatide guide for women covers the differences that matter beyond price.

Why do compounded semaglutide prices vary so much?
The medication itself is a small part of the spread. Five things drive most of it.
What the fee covers. Some programs quote one all-in number that includes the clinician, the medication and shipping. Others split the bill into a medication price and a separate membership. Alloy's semaglutide, for example, is from $169/mo all-in ($70 medication + $99 membership). A low headline can hide a second charge, so add the pieces before comparing.
Flat pricing versus dose-based pricing. Several programs charge the same at every dose, which protects you as your dose rises. Others start low and climb as you move up the titration ladder, so the first two months are the least you will ever pay.
Intro offers. A first-month price is common. MyFastRx charges $69 for the first month of semaglutide, then $139. The Edit charges $200 for the first month of semaglutide, then $259. Budget for the second month, not the first.
Prepay discounts. At least 15 of the 22 programs publish a lower monthly rate if you pay for three, six or twelve months up front. The discount is real, but you carry the risk: if the medication does not agree with you in week three, you may have paid for months you will not use.
Clinical extras. Lab work, a dedicated women's-health clinician, hormone therapy alongside the GLP-1, or a dietitian all cost something. A program that includes them may be better value at a higher price than a bare prescription at a lower one.
What billing traps should you watch for?
The sticker price is only one of three numbers you need. The other two are the billing cycle and the commitment.
Four-week billing. Among the programs we review, 11 bill compounded semaglutide every 4 weeks rather than every calendar month, so they sit outside the index. That is 13 charges a year, not 12. At $199 every 4 weeks you pay $2,587 a year, against $2,388 for $199 a month. One program renews on a cycle shorter than four weeks, which means even more charges a year, and we left it out of the index because the "monthly" label does not describe it.
Prepay shown as the headline. Several programs advertise their 12-month rate as the price, in large type, and show the month-to-month rate only at checkout. If a figure seems low, look for "billed annually", "12-month plan" or "per month, paid up front" near it.
Starting dose only. "From $X" usually means the lowest dose. Ask what you will pay at the dose you are likely to stay on.
Cancellation terms. Check how much notice you need to cancel and whether a shipped vial can be refunded. Our checklist for judging a program before you pay has the questions to ask.
How does compounded semaglutide compare to brand Wegovy or Ozempic?
Brand prices have dropped sharply since late 2025. In October 2026, NovoCare Pharmacy lists the Wegovy pill at $149 a month at 1.5 mg, $199 a month at 4 mg and $299 a month at 9 mg and 25 mg. New self-pay patients pay $199 for the first two fills of the Wegovy pen at the starting doses, then $349 a month. Ozempic pens follow the same $199 then $349 pattern, with $499 for the 2 mg dose.
| Option | What you pay per month (self-pay) |
|---|---|
| Compounded semaglutide, median program | $177 |
| Wegovy pill (brand, oral) | $149 to $299 by dose |
| Wegovy pen (brand, injection) | $199 for two starter fills, then $349 |
| Compounded tirzepatide, median program | $239 |
| Zepbound vial or KwikPen (brand) | $299 to $449 by dose, with on-time refills |
So the gap that made compounding the obvious choice in 2023 and 2024 has narrowed. At the median, compounded semaglutide still saves about $172 a month over the Wegovy pen, but the brand Wegovy pill at 1.5 mg costs the same as or less than the median compounded injection. Our guide to TrumpRx, LillyDirect and NovoCare prices lists every brand price by dose, and our compounded vs brand-name guide covers the quality and safety trade-off.
Is compounded semaglutide still legal in 2026?
Yes, within limits. A licensed pharmacy may compound a medication for an individual patient with a prescription, but compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality before they are sold2.
What changed is the shortage. During the semaglutide shortage, the FDA let pharmacies make what were effectively copies of Wegovy and Ozempic. On February 21, 2025, the FDA determined that the shortage of semaglutide injection products was resolved. The grace period for state-licensed pharmacies ended in April 2025, and for larger outsourcing facilities on May 22, 2025. The tirzepatide shortage had been declared resolved earlier3.
Since then, a pharmacy can generally compound semaglutide only when the product is meaningfully different from the approved drug for that patient, such as a different strength, an added ingredient or a different form, or in small numbers. The FDA describes a product with the same active ingredient, a similar strength and the same route as "essentially a copy", and it does not intend to act against a compounder that fills four or fewer such prescriptions a month3. That is why many programs now sell semaglutide with vitamin B12 added, or at a custom dose.
Enforcement has tightened. In February 2026 the FDA said it intends to restrict the active ingredients used in non-approved compounded GLP-1 drugs and warned companies that they cannot call compounded products generic versions of approved drugs or say they use the same active ingredient4. In April 2026 it proposed excluding semaglutide, tirzepatide and liraglutide from the list of bulk substances that outsourcing facilities may compound from5. Prices and availability may shift again as those rules settle.
Is compounded semaglutide safe?
It can be prescribed and used safely, but it carries risks that the brand product does not. As of May 31, 2026, the FDA had received 990 reports of adverse events associated with compounded semaglutide and more than 730 with compounded tirzepatide. It has also received reports of hospitalizations linked to dosing errors, when patients measured the wrong amount from a vial or a clinician miscalculated a dose2.
The FDA's practical advice is worth following: do not use an injectable that arrives warm, discard a multi-dose vial within 28 days of first use even if medication remains, and use a new sterile needle and syringe for each dose. It also warns against any seller that claims a compounded drug is the same as an FDA-approved one2.
What does the full monthly cost look like for women?
For many women the GLP-1 is one line in a larger budget. A realistic monthly figure includes:
The medication and visit fee. The figures above.
Contraception. GLP-1 medicines are not used in pregnancy, and tirzepatide can make the pill less reliable for four weeks after starting and after each dose increase. If you switch methods, that may cost something. Our guide to GLP-1 medication and birth control explains the rule.
Labs. A baseline panel before you start, and repeat checks during treatment, may or may not be included. Ask before you sign up. Our guide to labs and monitoring for women lists what a careful program checks.
Hormone therapy. If you are in perimenopause or menopause, some programs offer hormone therapy in the same plan, which can be simpler and cheaper than a second clinic. Our menopause board shows which programs do.
If you have PCOS, ask whether the clinician has managed insulin resistance and irregular cycles before; the PCOS board is a good place to start. For a broad comparison, see our GLP-1 board for women.
How should you compare prices?
Line up three numbers for every program: the price at your likely maintenance dose, the billing cycle, and the commitment. Then add anything billed separately. A program with no commitment and labs included can cost less over a year than one advertising a low 12-month prepay rate that bills far more if you go month to month.

Frequently asked questions
How much does compounded semaglutide cost per month?
Across 22 telehealth programs we review that publish a firm month-to-month price, compounded semaglutide costs $129 to $297 a month, with a median of $177. Half charge between $150 and $214. First-month offers and prepaid plans can look lower than what you pay from month two.
How much does compounded tirzepatide cost?
Across 21 programs with a firm monthly price, compounded tirzepatide costs $119 to $349 a month, with a median of $239. Of those programs, 6 charge $300 or more.
Why is compounded semaglutide cheaper than Wegovy?
Compounded products are not FDA-approved and do not carry the cost of the manufacturer's trials or brand pricing. The gap has narrowed: in October 2026 the brand Wegovy pill costs $149 to $299 a month through NovoCare Pharmacy, and the Wegovy pen $349 after two lower-priced starter fills.
Is compounded semaglutide legal in 2026?
Yes, from a licensed pharmacy with a prescription for an individual patient. Since the FDA declared the shortage resolved in February 2025, pharmacies can generally compound it only when the product differs meaningfully from the approved drug for that patient, or in small numbers.
What does billed every 4 weeks mean?
It means 13 charges a year instead of 12. A $199 price billed every 4 weeks costs $2,587 a year, compared with $2,388 at $199 a calendar month.
Is it worth prepaying for several months?
Only if you are confident you will stay on treatment. At least 15 of the 22 programs in our index offer a lower monthly rate for three, six or twelve months up front, but you may not get money back if the medication does not suit you.
Does the price include labs and clinician visits?
It varies. Some programs quote one all-in price; others add a separate membership or bill labs on their own. Add every charge before comparing programs.
